All athletes youth - professional must submit completed and signed forms prior to training with us.

 If you are a new athlete please download, complete, and sign the

ENROLLMENT/MEDICAL FORM.

 Every athlete is required to have a authorization form on file.  Please download, complete, and sign the

CREDIT CARD PAYMENT/AUTHORIZATION FORM.

After completing the appropriate forms please bring with you on your first visit.
